Patient Eligibility

We operate under the Florida Department of Health’s Volunteer Health Care Provider Program and because of this, patients are subject to a financial eligibility screening.

Individuals and families without health insurance and income below 200% of the Federal Poverty Level are eligible for free medical services. Click here to determine your FPL percentage category.

For more information, please contact the clinic. This screening process is quick and easy, and can be completed on the same day as a physician visit.

Patients do not need to bring any documentation in order to complete this screening.
